Participation Agreements | 2007-2008 Fiscal Year Statistics

The Order Form has been updated as of December 23, 2008. 

You can use Lands' End Scrip Gift Cards at Sears and Kmart with a current bonus benefit of 17%!

Please note, effective immediately, King's Family Restaurant has a plastic Scrip Gift Card for $10.00 rather than the $5.00 paper Scrip Gift Certificate; the benefit remains unchanged at 20%.

Participation Agreements:

If your child(ren) attend either the Saint Rose of Lima Parochial School, Bishop Guilfoyle High School, and/or Saint Rose of Lima Religious Education, you will need to complete a Participation Agreement.  The Agreement is effective May 1, 2008 through April 30, 2009. If you happen to have children attending more than one School during the effective time-period, you only need to complete one Participation Agreement, in this order:  Saint Rose of Lima Parochial School is first.  If you have no children attending Saint Rose of Lima Parochial School, next is Bishop Guilfoyle High School.  If you have no children attending Saint Rose of Lima Parochial School or Bishop Guilfoyle High School, then you would complete the Saint Rose of Lima Religious Education form.
